   You are required to come along with the following under listed documents:

  Application form

 JAMB Result Slip

 O’Level Result Slip

 Two passport photographs

 Letter from a religious leader

 Sponsorship letter

 Birth Certificate

State/ LGA Certificate of Origin, and your N2000 Screening Fee receipt.

VENUE: University’s Campus, Omu-Aran, Kwara State.

DATES: 3^rd, 10^th, 17^th, and 24^th of June, 2017.
